2. Have the responsible party/team manager/captain fill-out Step1.
3. Have each player enter their info in Step2. Each player must sign. If player is a minor, a parent or guardian must sign. Also, if there's any medical condition we should be aware of, please let us know.
4. Payment of fees should be noted in Step3. Zelle is the preferred method of payment. Please Zelle only your team's total fee.
5. Have the responsible party/team manager/captain sign and date in Step4.
6. Submit this form by email to: pbachicago23@gmail.com or in person to one of the PBA Chicago officials.

Team rosters are COMPLETE. Click on the link above to see your team roster along with your team's payment history. Please reach out to your teammates if you have questions about it. Emil was instrumental in creating these.
Once rosters are finalized, that's it - no more changes. If you have player injuries, you will have to make due with the players you have. If there are questions, please let us know. Thank you.

On Teams:
Based on players' requests, extensive efforts have been made to "even out" the teams.
In doing so, there became a need to add younger players (who wanted to play) to the rosters so that there are enough players and teams for this season.
On Fees:
We can't stress enough the importance of paying your team fees on time. We've done our part in keeping this league going. We just need you to do your part by paying your team fees now.
DESPITE OUR EFFORTS, A MAJORITY OF THE TEAMS HAVE NOT MADE FULL PAYMENTS!
